How to treatment plan for congenitally missing teeth is always a challenge. This new article describes an option for managing congenitally missing lower second bicuspids. This patient is congenitally missing both lower second bicuspids. The decision was made to close the space of the missing teeth. Consequently, the lower second deciduous molars were hemisected (first picture). In the second picture the amount of mesial drift of the first permanent molar after 11 months is quite remarkable. We will be extracting the remaining half of the lower second deciduous molars soon and allow further space closure. When all permanent teeth erupt, we will place braces and idealize tooth position and occlusion.
